William Tolley, 42, responded to a fire that broke out on the second floor of a multi-family home on 1615 Putnam Avenue in Ridgewood around 2:00pm. A city official tells ABC News that the first responder was attempting to get to the top bucket section of Ladder 135 from the roof, when he fell to his death.

Witness also tell the news station that the New York firefighter tried attaching his safety harness to the truck, but lost his balance as the basket shook and he fell to the sidewalk.
